Health Forms and Documentation for New Students:
Vaccine requirements for the State of Pennsylvania, as related to the college or university student, state the following:
"All college and university students who live in dorms are required to receive one dose of the Meningococcal vaccine. If a student does not wish to receive the vaccine, a waiver must be signed. In the case of the student being a minor, the waiver must be signed by the parent or legal guardian."
