All nursing students are required to submit the following documentation of reasonably good health and disease prophylaxis to be submitted to the Department of Nursing and Health:
1.) Health evaluation including specified laboratory results (form provided
by the Department of Nursing and Health).
2.) Health and Immunization Status form documenting immunization dates for poliomyelitis,
measles, mumps, rubella, varicella, tetanus, Hepatitis A, B, and C, and diphtheria.
3.) 2-Step Tuberculin Sensitivity Test
4.) Hepatitis B Vaccination (series of three injections).
5.) Child Abuse Check
6.) Criminal Record Check
7.) Federal Fingerprinting Check
Titers needed (all titers should report positive/immunity to the disease):
1.) Rubeola
2.) Rubella
3.) Varicella (if unable to provide documentation of immunization or having had the disease)
4.) Hepatitis (HBsAb)
Tests and Documents to be completed yearly:
1.) Single-Step Tuberculin Sensitivity Test
2.) RN License
3.) Liability Insurance
4.) CPR certification
Drug testing may be required of the student depending on clinical site placement. |
